At the review date, assess the outcomes by repeating the screening tool (STEP 3) and adjust your strategies as needed. Then repeat the process.

When you complete STEP 5 by pressing the submit button below, you will be emailed a copy of your answers up to this point. You should save this information under your organisation's file for the young person in question. Once you receive the email, all submitted answers will be erased from our system.
